Flow forms

Flow Forms puts your new patient forms on the patient’s phone and on an iPad for your team, then files them in Cliniko for you. Nobody needs to re-type a paper form, and your practitioner has the history before the patient walks in.

How it works

• Paste a Cliniko API key from an Administrator account. Flow Forms finds your clinic, timezone, and team, getting you set up in about ten minutes.
• Add your logo, colour, cover picture, and greeting. Start from your country's standard form (including Australian Medicare/DVA, New Zealand ACC, and more). Relabel, reorder, hide, or add questions, and use your own consent wording.
• Send forms however suits you: include the link in your Cliniko appointment reminders, email it directly, use a QR code at the desk, or hand over a reception iPad that lists today's appointments.

What syncs with Cliniko

• New patients are created, and existing patients are updated with their name, date of birth, contact details, address, and any other mapped fields.
• Answers are filed directly into the patient's notes, including their GP, medical conditions, medications, injury details, body chart, and pain scores.
• The signed PDF, complete with the body chart, consent, and signature, is saved to the patient's files.
• Flow Forms never alters your appointments, invoices, or treatment notes.

• A simple, one-question-at-a-time interface designed for easy use on phones, iPads, and computers.
• An interactive body chart that patients can tap, covering both front and back.
• Outcome measures (such as NDI, ODI, LEFS, SPADI, and PSFS) sent before a follow-up and automatically scored for you.
• A secure inbox for any form that could belong to multiple patients, ensuring a walk-in never overwrites the wrong record.
• Monthly reporting on form counts and submission methods.

Privacy

A form that files itself cleanly is not kept by Flow Forms. Any form waiting to be filed is encrypted and automatically deleted after 14 days. Your Cliniko key is encrypted and securely locked to your clinic.

Pricing

One plan with every feature, priced by how many active practitioners you have in Cliniko. Admin and reception logins are free. Pricing starts at NZ$25 a month for one practitioner, up to NZ$189 for 26 or more (excluding GST). Start with a 14-day free trial, no credit card needed.
