ClientForms pairs clinically validated instruments with a patient experience designed from the ground up to reduce cognitive load. Every decision about pacing, progress, and how much a patient holds in working memory at once is made deliberately, so responses come from patients who are thinking clearly. Patients respond more carefully, and you get more accurate data to work from.

The system is built for psychology practices, occupational therapists, counsellors, and creative arts therapists. The library covers depression, anxiety, trauma, functional impairment, autism, and ADHD, and it's used day-to-day for Medicare mental health treatment plans, NDIS progress reporting, and routine outcome monitoring across sessions.

How the Cliniko integration works

• Your Cliniko patient list syncs into ClientForms automatically, so practitioners never re-enter contact details or create duplicate records.
• You email a patient a form link before their session, or show an in-app QR code in your waiting room so they complete it on their phone while they wait. No app download or patient login.
• When the patient finishes, the scored result is written back into Cliniko as a treatment note, with a PDF of the full report attached and a link back to ClientForms for anything you want to examine further.
• Open the ClientForms button from any patient's Cliniko sidebar and a panel opens with their latest scores, severity, subscale breakdown, and how those results have moved across previous visits, without having to scroll back through old notes. This is ncluded on every plan.

• Sections capped at 5 to 7 questions, based on working memory research, instead of scrolling walls.
• Pacing cues and adaptive time estimates, so patients with attention and executive function differences always know how much is left.
• Calm defaults means no flashing, no clutter, no unnecessary motion.

Data residency and security

ClientForms runs two regional data environments. Practices using the Cliniko integration are hosted on Australian servers, with patient data stored in Australia and never routed offshore. Practices in the United States are hosted in a separate US environment designed to meet North American healthcare data requirements. The Cliniko integration itself is currently available for Australian and New Zealand practices only, with more regions to be added down the track.
